Entry Fee - $11
Fee includes the official Newport Mesa Soccer Classic T-Shirt, as well as administrative fee.
To ensure the tournament is organized in a timely manner, entry fees must be received no later than Sunday, April 20th. 
 
Photograph Disclosure and Consent:
By selecting YES above, you agree that your child participating in the Newport-Mesa Soccer Classic event may be photographed and/or video taped and waive all rights to the said photos or videos. Photos taken at the Newport-Mesa Soccer Classic event can be used for commercial purposes without any compensation or further consent.
 
Waiver:
By selecting YES above, you are indicating the following:
I understand that the Newport-Mesa Soccer Classic event involves physical activity, including without limitation, physical contact, which has inherent risks, including injury that may result to my child. On behalf of myself and my child(ren), and all respective heirs, I hereby agree to assume all risks associated with participating in the Newport-Mesa Soccer Classic, and waive and release and any all claims for damage or otherwise against Costa Mesa United and/or any of its affiliated entities, representatives, or assigns in connection with my child(ren)'s participation in the Newport-Mesa Soccer Classic event.
 
Your typed name serves as a signature approving your child(ren)'s participation in the Newport-Mesa Soccer Classic event.
